When the Doctor investigates a mind-altering machine at Stangmoor Prison, he uncovers a sinister plot involving a Chinese delegate's murder, a nerve gas missile, and the manipulative machinations of his nemesis, the Master, threatening global peace.

The Doctor and Jo Grant arrive at Stangmoor Prison, where Professor Kettering has been found dead, seemingly drowned in a dry room. The Doctor quickly deduces that the Keller Machine, designed to extract negative impulses from criminals, is responsible. He discovers that the machine is not only deadly but also capable of drawing out a person's deepest fears and using them against them, as it does to him, forcing him to relive a past trauma involving a world engulfed in flames.

Meanwhile, UNIT, led by Brigadier Lethbridge-Stewart and Captain Yates, is grappling with heightened tensions at a peace conference following the death of the Chinese delegate. The Doctor is urgently called back to London to assist. Sergeant Benton is assigned to tail Captain Chin Lee, a suspicious aide to the new Chinese delegate, but loses her after experiencing a sudden, debilitating headache, suggesting a connection to the Keller Machine.

As the Doctor briefs the Brigadier on the dangers of the Keller Machine and its possible connection to the recent events, Yates prepares to escort a nuclear missile, dubbed "Thunderbolt," for disposal at sea. However, it is revealed that Yates himself is under the control of the Master, who has infiltrated UNIT disguised as a telephone engineer, manipulating events to his own ends. The Master orchestrates a plan to steal the missile and further destabilize the peace conference.

Back at Stangmoor Prison, Jo disobeys the Doctor's orders and investigates the Keller Machine. She visits Barnham, a patient who underwent treatment with the machine, finding him unnervingly passive and devoid of negative emotions. A prison riot erupts, instigated by prisoners seeking to escape and avoid being subjected to the machine's effects. During the chaos, prisoners take hostages, including Doctor Summers, while using firearms purloined from the cleaning closet.

In London, Chin Lee, mind-controlled by the Master, lures the American delegate, Senator Alcott, into a trap. The Brigadier learns of Chin Lee's reappearance and her contact with Senator Alcott. With the peace conference on the brink of collapse and the Master's plans nearing fruition, the Doctor and the Brigadier find themselves racing against time to prevent a global catastrophe. They must expose the Master's deception, stop the theft of the Thunderbolt missile, and rescue the hostages at Stangmoor Prison before it's too late.

Act 2

Following his arrival in London, the Doctor successfully convinces the Brigadier to support a ban on the dangerous Keller Machine, acknowledging its destructive potential. Their subsequent meeting with the new Chinese delegate, Fu Peng, highlights the Doctor's unexpected fluency in Hokien, creating a temporary, if somewhat comical, diversion from the pressing diplomatic tensions. Meanwhile, back at Stangmoor Prison, Jo Grant disobeys the Doctor's strict instructions to remain safe. She investigates Barnham, a patient treated by the Keller Machine, discovering that the device has rendered him devoid of negative emotions, effectively turning him into an "idiot or a saint," underscoring the machine's extreme and unpredictable effects. This act of disobedience places Jo directly in harm's way as a prison riot erupts. Prisoners, led by Mailer and Vosper, seize control, taking officers hostage and acquiring a firearm from a cleaning closet, escalating the localized threat within the prison. The Master's malevolent plan accelerates as he hypnotizes Captain Chin Lee, compelling her to assassinate the American delegate, Senator Alcott, that very night, directly threatening the fragile peace conference. The prison subplot intensifies further when Jo and Doctor Summers are taken hostage by the rioting inmates, placing Jo in immediate, life-threatening danger.
