The Doctor and Leela find themselves in a desperate battle against the Nucleus, a highly intelligent and adaptable alien virus that has taken over the Bi-Al Foundation. The Nucleus, with its ability to evolve and mutate, has created an army of giant, invulnerable creatures to spread its influence across the Solar System. The Doctor and Leela must navigate through the Foundation, evading the Nucleus's minions and working to find a cure for the infected humans. Along the way, they discover that the key to defeating the Nucleus lies in Leela's own biology, which holds the immunity factor that can be used to create a counter-virus. As the Nucleus prepares to hatch on Titan, the Doctor and Leela race against time to reach the planet and inject the counter-virus into the breeding tanks, preventing the Swarm from spreading. In a final showdown, the Doctor uses his intelligence and cunning to outwit the Nucleus and ultimately destroy it by using a methane and oxygen mixture to create an explosion that eradicates the alien threat.

The narrative opens with the Doctor recovering from a viral infection, the scales on his skin vanishing as the Nucleus, now fully grown into a prawn-like creature, reveals its malevolent plan. Professor Marius, under the Nucleus's control, declares his service to the entity, which intends to breed on Titan, transforming its microscopic swarm into an invincible, macro-world army that will end the age of man. Leela, disguised in a nurse's uniform, orchestrates the Doctor's escape from the isolation ward, cutting his restraints and using the gurney as a diversion against Lowe. The Nucleus, prioritizing its journey to Titan, orders Marius to recapture the Doctor later. Trapped in the TARDIS, immobilised due to the dimensional stabiliser remaining in the ward, the Doctor dispatches K9 to incapacitate Marius and retrieve the crucial device. K9 successfully "poleaxes" Marius, allowing the Doctor and Leela to return to the isolation ward. There, the Doctor examines Leela's blood, discovering a unique antibody – an immunity factor – that he deduces was passed to him via his clone. He formulates a strategy: isolate, analyse, duplicate this antibody, and inject it into Marius to cure him, then scale up production to combat the Nucleus directly. Meanwhile, Lowe and the Nucleus race towards Titan, accelerating their spawning schedule. Back at the Foundation, Marius, now cured, assists in cultivating the powerful C531 antibody strain. Leela proposes simply blowing up Titan, a suggestion the Doctor dismisses due to the lack of suitable weaponry. With the potent antibody strain prepared, the Doctor explains his refined plan to inject it into the breeding tanks on Titan, intending for it to microscopically attack the Nucleus. Borrowing K9 from Marius, the Doctor and Leela dematerialise in the TARDIS, embarking on their critical mission to Titan.

He devises a diversion, sending K9 to decoy a guard, drawing the hostile away. While K9 executes his mission, Leela takes up a position near a large oxygen tank, preparing for direct combat. The Doctor, meanwhile, attempts to reach the hive with the box of antibodies, but Lowe intercepts him, causing him to drop and lose the crucial antidote. In a moment of quick thinking, K9, despite his low power, shoots Lowe, allowing the Doctor to push the infected man into the hive, where he is presumably consumed by the nascent swarm. K9 then completely runs out of power, forcing the Doctor to drag the inert robot along. Leela, demonstrating her hunting prowess, ambushes and kills Safran by stabbing him in the neck, eliminating another of the Nucleus's human agents. With the antibody plan now defunct, the Doctor improvises a new, more destructive solution. He opens the valve of a large oxygen tank, allowing the gas to flood the area, and then wires the hive door shut, trapping the Nucleus inside. He pulls a tube from the side of the hive, further manipulating its environment. As the Nucleus taunts him, the Doctor attaches a blaster to the hive door handle, creating a volatile booby trap. He, Leela, and the disabled K9 then race back to the TARDIS, making a narrow escape as the Nucleus begins to force its way out of the hive.
