The test tube is the vessel for the Doctor’s experimental vaccine, handed to him by the Monoid without prompting. The Doctor pours the concoction into it, then uses it to apply the vaccine to Steven’s arm. Its role is dual: a medical tool and a metaphor for the gamble being taken.
