Act 1

Solon, a reclusive neurosurgeon, establishes his macabre ambition on the desolate planet Karn, rejecting an insectoid head as unsuitable for his grand project. Simultaneously, the Doctor and Sarah Jane's TARDIS is forcibly diverted to Karn, where they discover a landscape littered with wrecked spacecraft and a recently murdered alien. While exploring, the mystical Sisterhood of Karn, led by Maren, observes their arrival. Maren expresses deep concern over the diminishing Flame of Life, the source of their Elixir, and identifies the Doctor as a Time Lord, fearing he has come to steal their sacred substance. Seeking shelter from a storm, the Doctor and Sarah arrive at Solon's castle. Solon immediately fixates on the Doctor's head, expressing unsettling admiration. The Doctor, recognizing a bust in Solon's parlour as the notorious Time Lord Morbius, grows suspicious. Solon then drugs their wine, incapacitating both the Doctor and Sarah Jane, thereby securing his desired specimen for his sinister experiments.

Act 2

With the Doctor and Sarah Jane incapacitated, Solon triumphantly declares his plan to transplant Morbius's brain into the Doctor's body, envisioning this as his 'great triumph' that will command the universe. Sarah Jane, feigning unconsciousness, overhears Solon's entire macabre scheme, including his instruction to Condo to kill her. Solon dismisses Condo's concerns about the Time Lord's power, labeling them 'spineless parasites' who betrayed Morbius. He meticulously plans the delicate surgical procedure, emphasizing the need for precision and skill. As preparations for the operation begin, Solon realizes the castle's power generators require repair to provide adequate lighting and power for his instruments. He and Condo depart to address this issue, leaving the unconscious Doctor unattended on the examination couch in the laboratory, creating a precarious window of opportunity.

Act 3

Following Solon and Condo's departure to repair the generators, a mysterious mist envelops the unconscious Doctor on the examination couch. The mist causes the Doctor to vanish completely, leaving the couch empty. Meanwhile, Sarah Jane, having successfully feigned unconsciousness, fully awakens and cautiously begins to move, tracking Solon and Condo's path through the corridors. She enters the laboratory, expecting to find the Doctor still captive, but discovers the examination couch vacant. Confronted with the Doctor's inexplicable disappearance, Sarah Jane's objective shifts from passive observation and escape to actively searching for her missing friend and understanding the new, immediate threat. She emerges from the shadows into the corridor, now committed to an urgent, independent investigation.
