On a planet controlled by the Happiness Patrol, the Doctor and Ace must stop the ruthless leader Helen A from imposing forced happiness and eliminating those who resist, while also navigating the complexities of a society that values superficial joy over genuine emotions.

The Happiness Patrol, led by the tyrannical Helen A, enforces a regime of forced happiness on a planet. The Doctor and his companions, including Ace and Susan, intervene to stop Helen's brutal methods of maintaining control, which include execution squads and the 'Happiness Patrol' section F. As the story unfolds, the Doctor and his friends encounter various allies, including Earl and Trevor, and face off against Helen's minions, including Daisy K and Priscilla P. A significant shift in power dynamics occurs when Priscilla, now back on duty, finds herself guarding Daisy K, who is subsequently released by Helen A via a screen broadcast, highlighting Helen's desperate attempts to maintain control. Within the pipes, the Doctor and Ace encounter the 'little people' and then face the formidable Fifi, revealed as a ruthless Stigorax. Helen A, informed of the Doctor's presence in the pipes, deliberately leaves Fifi to hunt him, intensifying the subterranean threat. The group navigates treacherous sections of crystalline syrup, which Earl identifies as highly unstable. Daisy is then stripped, gagged, and tied, while Susan and Earl begin broadcasting anti-Helen A messages, effectively dismantling the propaganda machine. The Doctor then confronts Helen A on the dark, quiet streets, where she attempts to flee to a scheduled flight. This confrontation marks the narrative's climax, transforming into a profound philosophical debate. The Doctor challenges Helen A's ideology of forced happiness, arguing that true happiness is meaningless without the contrasting experience of sadness, symbolizing this duality with a two-sided coin. Helen A remains defiant, clinging to her vision of a world without sadness, a place of control and absence of compassion or love. The emotional turning point for Helen A arrives abruptly and powerfully: she discovers her beloved pet, Fifi, dying on a manhole cover. Overcome with grief, Helen A finally breaks down and cries, experiencing genuine sadness and compassion for the first time, shattering her own facade of enforced joy.
