When the Doctor and his companions Giuliano and Sarah become embroiled in a sinister plot by Count Federico and the cult of Demnos, they must navigate treacherous catacombs, ancient temples, and palace intrigue to prevent a catastrophic ritual from unfolding.

In the midst of political turmoil and ancient mysticism, the Doctor, Giuliano, and Sarah find themselves entangled in a complex web of deceit and power struggles within the city. The story begins with the Doctor's encounter with the cult of Demnos, led by the enigmatic and sinister Hieronymous. Act 1

The Doctor and Giuliano narrowly escape Federico's soldiers and the encroaching Brethren in the catacombs, but Sarah disappears during the chaos. Federico, enraged by Giuliano's continued evasion, intensifies his hunt, ordering a city-wide search and vowing Giuliano's death. Hieronymous, meanwhile, prepares for the ritual, spreading prophecies of Demnos's return and guarding the sacred temple. The Doctor and Giuliano continue their search for Sarah through the extensive catacombs, eventually locating her. However, the Doctor immediately notices her strange, vacant eyes, which raises his suspicion about her altered state, hinting at the manipulation she has undergone. He deduces her mind has been taken over, likely by Hieronymous, and prepares for what he knows will be a dangerous confrontation.

Act 2

The Doctor confronts Hieronymous in his chamber, where Sarah, under hypnotic control, attempts to stab him with a poisoned needle. The Doctor disarms Sarah, breaking her trance, and then fends off Hieronymous, who is subsequently captured by Rossini and Federico's guards. Simultaneously, Federico's men capture Giuliano and imprison him in the palace dungeons alongside a tortured Marco. Federico, believing his path to dukedom is clear, gloats over his perceived victory and prepares for the execution of Giuliano and Marco. The Doctor, now also imprisoned, attempts to convince Federico that Hieronymous, not Giuliano, poses the true, imminent threat. He reveals Hieronymous's connection to the powerful Helix energy and warns of the catastrophic consequences if the ritual proceeds, attempting to shift Federico's focus from petty political ambition to the existential danger facing the entire city.

Act 3

Hieronymous, having escaped capture, gathers his believers in the temple and begins the ritual to summon Demnos, channeling the powerful red Helix energy. In the dungeon, Federico, initially dismissive, is eventually swayed by the Doctor's urgent warnings about the catastrophic potential of Hieronymous's ritual. The Doctor emphasizes that the cult leader's actions threaten not just Federico's ambitions but the entire city. Convinced of the dire stakes, Federico orders the Doctor unchained, agreeing to go to the temple to investigate the claims himself. However, Federico leaves Sarah, Giuliano, and Marco as hostages, giving Rossini strict orders for their execution if he does not return within the hour. The Doctor and Federico depart for the temple, heading directly into the unfolding ritual and setting the stage for the story's climax.
